A claim form, a pre-wash check sheet, a policy sign and a claim log, printed with your wash’s name on them.

What to print
How this works
The claim form asks for the same details as OpsConsole’s online claim form, so a paper claim can be typed in later with nothing missing. The signature and office boxes are for your own records.
These are starting points, not legal advice. Have your insurer look at your forms and your policy before you use them.
What each printout holds:
The claim form: the customer’s name, phone, email and address, their visit, their vehicle, what happened, where the damage is on the car and whether they have photos, then a signature and office-use boxes.
The check sheet: the car drawn from both sides and the top, a tick list (glass, trim, mirrors, aftermarket parts, roof rack, existing scratches or dents) and the attendant’s initials.
The policy sign: your wash’s name over your own policy.
The claim log: one page of rows for date, customer, plate, what happened, status and follow-up.
